Unfortunate Design Fails That We Can Learn From

Stairs:
Most of us, at some point in our lives, have missed a stair or thought we had one more stair to go. One wrong stepping movement can land you on your tush. It seems to me that whoever designed the stairs below did not think too much about the safety of those who would use them. This emphasizes how important choosing the correct size, shape, placement, material, and pattern for a staircase or ramp is. I really don't understand how some of these even got built. Shouldn't the contractor have been asking some questions like, "Hey, did you mean to draw this stairway to nowhere?"or,"Why do you have us installing a door on the side of the staircase?" We may never know, but let's make sure this doesn't happen again.
Functionality is Key:
Let's all remember to stop and think about the functionality of our designs before we start building so things like the following don't happen to us:
